Reporting to the Associate Director, Care Coaching, you will support the work of Care Management staff. You will provide daily leadership and mentoring to staff. You will deliver services in a manner that expresses Humana values and the Family Care program values of access, choice, quality, and cost effectiveness. Main Responsibilities: Mentor and support the work of Field Nurses and Care Coaches (or IDT) to ensure the Family Care benefit is delivered in compliance with the DHS contract.
