Inpatient Social Worker | Per Diem

Summary Under the direct supervision of the Director of Care Management and the Lead Social Worker II, The Social Worker I is consulted to provide assessment and referral for services related to the complex care needs of the patient and family for adjustment to illness, crisis intervention and to preserve patient's self determination in the decision making process. Education/Experience 3 to 5 years of Clinical Social Work experience required. Acute Care Social Work experience preferred. Must have either Bachelor's degree in Social Work or related field.
