Summary Under the general direction of the Medical Director and the Practice Manager, the Palliative Care Nurse Coordinator will work closely with the palliative care providers, other providers, care managers and nurses to coordinate comprehensive palliative care services. The palliative care nurse coordinator acts as a resource and support to patients, families and staff for palliative care and end of life issues. Certification, Registration, and Licensure Required: Current NH RN license; Certification in Palliative Care (CHPN) within one year of hire. Qualifications and Competencies: Strong organizational, communication, and interpersonal skills. Knowledge of palliative care principles, end-of-life issues, and healthcare systems preferred.…
