Location: Toledo Hospital Department: Care Navigation Acute Weekly Hours: 0 Status: Per Diem Shift: Days (United States of America) Job Summary: As the Clinical Social Worker , you will provide proactive care management for high-risk patients and their families by addressing psychosocial issues that may affect medical care and health outcomes. You will interview patients, families and caregivers to assess personal and social adjustments and look for any needed interventions to mitigate risks. Once identified , you will deliver interventions and connect patients to appropriate resources .
