Department: 34000 Milwaukee South - Administration: Nursing Status: Full time Benefits Eligible: Yes Hou rs Per Week: 40 Schedule Details/Additional Information: Full Time Home Health Float Pool position based in WI supporting the southern patient service markets. Areas of work based on assignment. Typical schedule is Monday through Friday 8:00am to 4:30pm with some variability based on departmental needs. Every 4th weekend rotation required. Pay Range: $35.50 - $53.25 Major Responsibilities Complete scheduled patient visits and collaborate with the interdisciplinary team. Conduct comprehensive patient assessments and update plans of care as needed. Communicate treatment changes with physicians and the primary registered nurse.…
