Department: 05580 Lakeshore Southpointe - Patient Education and Nutrition Status: Full time Benefits Eligible: Yes Hou rs Per Week: 32 Schedule Details/Additional Information: This position is scheduled for 32 hours per week (4 days per week, 8 hours per day) during regular clinic hours and will be based across two clinic locations. Exact days, hours, and clinic assignments will be determined as part of the hiring process. Monday-Friday 7am-9am flexible start time, end times flexible between 330pm-5pm. Pay Range: $35.50 - $53.25 Major Responsibilities: Provides care following the National Standard for Diabetes Self-Management Education and Support (DSMES) as defined by the American Diabetes Association (ADA) or Association of Diabetes Care and Education Specialist (ADCES).…
