About this role

The Case Manager provides direct and indirect patient care and case management services to active-duty patients receiving inpatient behavioral health treatment. The role focuses on coordinating individualized discharge plans, facilitating multidisciplinary collaboration, and ensuring seamless transitions to appropriate levels of care. *Pay Range: $23/hr. to $25/hr. *Work Schedule: Monday to Friday Essential Functions : Collaborate with the multidisciplinary clinical team to address patient care concerns and barriers to discharge, actively participating in treatment planning and case discussions.
