About This Position Job Description: Patient Representative Purpose: Under general supervision, connects patients with necessary services and resources to satisfy medical and social services needs. Duties, Functions and Responsibilities: Essential duties and functions, pursuant to the Americans with Disabilities Act, may include the following. Other related duties may be assigned. Coordinates necessary medical services and resources for patients, and serves as a liaison between patients, clinic staff, various local, state and federal agencies, and external companies and programs. Works with physicians and other clinic staff to evaluate and identify resources or services needed for patients. Interviews and gathers information to assist patients with obtaining needed resources or services.…
