Summary Job Description: BHMG Pulmonary & Critical Care Job Summary: The Nurse Navigator serves as the primary point of contact for physicians and other healthcare providers, patients and their caregiver/family, and other resource-based entities, providing and accessing clinical and supportive care services offered within the multidisciplinary teams at Baptist Health Hardin and other entities as needed. The Nurse Navigator serves as an integral part of the care team within the Baptist Health Hardin Cancer Care Services department, coordinating effective and comprehensive services through the continuum of care for patients from the time of all breast-related diagnoses (cancer-related or suspicious findings), with treatment and support service options and through the recovery phase of care.…
