Borrego Health is seeking an experienced Full-time- Referral Care Coordinator for our Desert Hot Springs location. Referral Care Coordinator plays a key role as part of the clinical team. The Referral Care Coordinator is responsible for assuring continuity of care for those patients with a referral order, by coordinating timely specialty or ancillary appointments. The referral process is a multistep process and the referral care coordinator is responsible to make sure that the process is following its course.…
