About this role

SHARE (Supportive Housing Assistance to Reach Excellence) is a supporting housing service designed to assist persons with serious mental illness(es) in maintaining independent housing in the community. The case managers provide the support needed in each person's home environment and in the community. SHARE provides outreach and case management services to program participants. The clinician works with these individuals in the community to complete assessments and develop an Individualized Rehabilitation Plan (IRP). Additionally, the clinician provides clinical oversight for the case managers to support consumers.…
