About this role

POSITION SUMMARY: The Advanced Practice Provider (APP) reports to the Manager for CVI APPs and the ambulatory medical director. The APP works as an integral member of the advanced heart failure team working with a group of heart failure cardiologists, fellows, nurses, pharmacists, and medical assistants. Unique hybrid role incorporating inpatient and outpatient practice, caring for complex advanced heart failure patients. Participates in busy inpatient heart failure consult service as shared visit with a group of heart failure cardiologists.…
