Welcome to Montage Health’s application process! Job Description: The Nurse Practitioner (NP) is an expert in diabetes care. The NP is responsible for the delivery of diabetes care and education that involves compliance with evidence-based clinical standards, outcomes measurement (tracking, analysis, and reporting of clinical, financial, and satisfaction outcomes) and process design (leading initiatives to facilitate practice-based and organizational change). As a member of the glycemic management team, the NP provides direct patient care in the inpatient setting, through implementation of standardized procedures and works collaboratively with the medical team to establish priorities and to assess solutions for patients to ensure they receive the best care possible.…
