About this role

CarepathRx supports health systems and hospitals with pharmacy and infusion services designed to improve patient care . We are seeking a Case Management Analyst (RN) to coordinate admissions and ongoing care for patients receiving infusion therapy. You will work closely with patients, providers, pharmacists, home health agencies, and internal teams . Responsibilities Coordinate patient referrals and admissions. Confirm patients meet admission and clinical requirements. Communicate with patients, providers, pharmacists, home health agencies, and internal teams.
