About this role

Job Description: The Behavioral Health Community Health Partner (BH CHP) manages a member panel to support care coordination and social care for high-complexity members with acute behavioral health conditions. Responsibilities: Assess/Intake Conducts assessments and screening instruments, including those for behavioral health, home safety, and social needs, while adhering to established protocols and maintaining regular check-ins with members. Collaborates with psychiatrists, RNs, Behavioral Health Specialists (BHS) and external providers as needed for case review, care planning, and to develop and maintain member care plans.…
