About this role

JOB SUMMARY The Community Outreach Specialist (Admission Coordinator) interfaces with hospitals, skilled nursing homes, board and care homes and outpatient discharge planning services, and physician offices. Provides and acts as a community resource and public relations representative in facilitating safe quality healthcare in our communities. The COS is expected to generate appropriate patient referrals/ admissions by establishing and maintaining professional relationships with all referral sources. As well as responsible for completing all referrals. DUTIES & RESPONSIBILITIES Responsible to become a subject matter expert on Medicare Home Health, Signatures Care’s offerings, disease specific clinical criteria and evidence-based medicine.
