Job Description Full-time (40 hours/week) · Fully Remote Position This role requires working hours from Monday-Friday 9:30am-6:00pm EST. A bit about this role: This position is an amazing opportunity for an ambitious nurse practitioner to take on a new leadership role in the Transitions of Care (TOC) Program and to help develop the clinical excellence of the team.
