Building Location: Essentia Grand Rapids Clinic Department: 1008370 CHNA - EH SS Job Description: This position requires community health workers (CHW) to have a close understanding of the communities they serve. CHWs are specifically hired and trained to address behavioral and social determinants of health through patient-centered goal setting. CHWs reach beyond clinic walls to help patients meet their goals. CHWs creatively leverage patient, family, and community resources to address root causes of patient’s health concerns. CHWs work to increase access to medical, behavioral health, and community services.
