RN Hospice Clinical Liaison (Excellent Benefits Package, Very Competitive Compensation and Bonus Package and much more!) Position Responsibilities: · Develop, expand and maintain strong referral-source relationships across the assigned market. · Represent hospice services professionally with referral partners, patients, families and community partners. · Use clinical knowledge to educate referral sources, patients and families on hospice care, eligibility, services and transition planning. · Identify new referral opportunities and strengthen existing accounts through consistent outside field activity. · Work closely with internal clinical and operations teams to support appropriate referrals and smooth transitions into care.…
