About Us – The Case Management team initiates discharge planning with every patient upon admission and provides assistance with aftercare, medical equipment, care coordination, and other discharge needs. Case managers are assigned to individual units but also may follow patients with specific discharge needs to provide targeted, patient-centered care. The Case Management team collaborates with physicians, nurses, ancillary staff, patients, and families to coordinate excellent care.
