Location: Geisinger Wyoming Valley (GWV) Shift: Days (United States of America) Scheduled Weekly Hours: 40 Worker Type: Regular Exemption Status: No Job Summary: Full Time Day Shift 8am - 4:30pm Monday - Friday; REQUIRES: Two years' related experience and High School Diploma or GED Job Duties: Coordinates patient care and maximizes patient satisfaction and experience. Partners with care providers to serve as a liaison to ensure effective communication with patients. Provides discharge information. Partners with providers to ensure a smooth transition at discharge to home or other facilities as required. Ensures additional resources are made available for the patient as needed.…
