Job Summary The Care Resource Coordinator will provide linkage to care case management using a strength-based approach with people living with HIV/AIDS. Assists in the development and implementation of a recruitment plan for connecting HIV infected individuals recently diagnosed or lost to care at IDP. The individual also coordinates with case management partners/health care team assuring smooth, timely transition between sites of care. Provides ongoing client referrals to appropriate community resources and services based on client need and eligibility.…
