Job Summary The Community Health Worker collaboratively works with partners in the Grady Health System and community to provide comprehensive community outreach services and health education to high utilizers of Grady Hospital. QUALIFICATIONS Bachelor’s degree in public health, social services or related field is required. Experience providing health education is preferred.
