Night (United States of America) Licensed Practical Nurse- Hospice The Field Licensed Practical Nurse provides direct patient care to hospice patients in a variety of settings and levels of care under the direct supervision of an RN. Days/Nights/Weekends/Holidays as assigned to provide 24 hours a day, 7 days a week care assigned to a variety of teams/duties. Responsibilities include: • Providing care to the terminally ill patient • Reinforcing patient and family training and education • Performing patient assessment and documentation • Participating in the comprehensive Interdisciplinary Plan of Care • Participating in the Quality Improvement activities All services will be provided 24 hours a day, 7 days a week as assigned to a variety of teams and duties.
