Occupational Therapist Home Care Per Diem

Job Description: Position Summary : The occupational therapist is responsible for the implementation of standards of care for occupational therapy services and for adherence to the plan of care established by the physician. Position Responsibilities Include: Makes home visits to patients in designated geographic territories. Performs thorough evaluations of patients’ functional status, including muscle function, endurance, visual coordination, written and verbal communication skills and ADL and IADL performance. Evaluates the home environment for hazards or barriers to more independent living and identifies equipment needs. Continually assess and revise the occupational therapy care plan, and participate with other team members in the multidisciplinary care plan as appropriate.…
