The Care Coach 1 work assignments are often straightforward and of moderate complexity. You will report to Manager, Care Management Behavioral Health. The Field Care Manager, Behavioral Health employs a variety of strategies and techniques to manage a member's physical, environmental and psycho-social health issues. Resolve barriers that hinder care. Ensure patient is progressing towards desired outcomes by managing patient care through assessments and evaluations. May create member care plans.…
