Become a part of our caring community As a Telephonic Registered Nurse Care Manager, you will support patients by coordinating their care, assessing their healthcare needs, and helping ensure they receive the appropriate services and resources. You will be working fully remote and communicating with patients, caregivers, providers, and other members of the healthcare team. Your clinical experience will help you identify patient needs, provide education, and support safe care planning. You will manage care for patients with a variety of healthcare needs.
