About this role

Job Description: The Pulmonary Care Coordinator provides advanced care coordination, program navigation, and operational support for patients undergoing evaluation and treatment within a comprehensive pulmonary program. This role serves as a key liaison between patients, providers, and multidisciplinary teams to ensure timely progression through diagnostic evaluation, procedural interventions (e.g., right heart catheterization), and longitudinal care management. The Coordinator applies established clinical workflows, program protocols, and payer requirements to guide patients through complex pulmonary care pathways, support adherence to care plans, improve patient experience, and contribute to the growth of an integrated Pulmonary or Lung Institute model.…
