Job Description: The Transplant Assistant II - Transplant Navigator facilitates effective clinic organization and patient care by serving as a communication link between the patients, staff, physicians, hospital departments or other outside health care entities. Ensures compliance to CMS requirements for communication to families and donors. Develops and maintains systems for data management to meet requirements and for reporting to internal and external consumers of the information. Is responsible for navigating patients through multiple testing, consultations and / or treatments that patients receive in transplant through ongoing and extensive interaction with the patients and transplant coordinators.…
