About this role
Our client based in Lincolnshire is seeking experienced Social Workers to join their Mental Health Discharge Service. This is an excellent opportunity for skilled practitioners to support adults with mental health needs through timely and effective hospital discharge pathways, whilst also managing a community caseload and undertaking ongoing assessments and reviews. The successful candidates will manage a caseload of adults requiring assessment, care and support planning, and the coordination of services to facilitate safe and effective discharge from inpatient mental health settings.…
