MDS Coordinator (RN) – Clinical Care Planning & Assessments Locations (On Site): This position will support Cedar Falls (Deery Suites, Martin Suites), Grundy Center (Creekside), and Madrid (Madrid Home). Western Home Communities is seeking a Registered Nurse (RN) MDS Coordinator to lead the MDS/RAI process, care planning, and resident assessments in a dynamic long-term care environment. This role is ideal for an RN who enjoys clinical coordination, regulatory compliance, interdisciplinary teamwork, and resident-centered care planning .
