About this role

Overview: The job will start at 2 to 3 8-hour shifts per week. By the summer of 2026 it will become full time, (40 hours) M-F 8:00am to 4:30pm. Job Description Summary: Ensures care continuity for patients and families and collaborates in the development, implementation, and evaluation of all program components. Job Description: Essential Functions: Develops, implements, and enhances special projects promoting inpatient/outpatient continuity of care and parent engagement. Ensures the provision of high quality, individualized, comprehensive, cost effective patient across the continuum.
