Hourly Pay Range: $30.46 - $45.69 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors. Position Highlights: Position: Discharge Planner - Float Location: LOH-Naperville Full Time- 40hrs/week Hours: Days 8:00-4:30 What you will do: Floats to all inpatient units and demonstrates ability to quickly shift between clinical responsibilities and discharge planning. Conducts psychosocial assessments, family sessions, group and carries a therapy caseload as needed. Demonstrates a comprehensive knowledge of community resources (12 step support meetings, recovery home and residential treatment facilities) and medical allied staff expertise to assure an appropriate discharge liaison is implemented.…
