Position Highlights: Position: Mental Health Clinical Therapist I Location: LOH Plainfield Full Time Hours: Days- 8:00-4:30 What you will do: Conducts psychosocial assessments by interviewing the patient and their significant others to obtain history and identify preliminary issues for treatment focus. Contacts appropriate previous and current tertiary caregivers to obtain relevant collateral clinical data that may be needed for the patient’s coordination of care, treatment planning and discharge planning.
