About this role
SUMMARY
The role of the Health Care Navigator is to drive post-acute and long-stay census results for the Toledo Ohio Otterbein ministries. The position will work on a complementary basis with the Senior Lifestyle Communities; Sunset House, Sunset Village, Pemberville & Marblehead– along with Otterbein Small House Neighborhoods; Monclova, & Perrysburg. With a specific focus on: Otterbein Perrysburg, Otterbein Monclova, Otterbein Sunset House, Otterbein Sunset Village, and Otterbein Pemberville. With a targeted emphasis on the Toledo Ohio Hospital Systems (key accounts- Promedica, Mercy, UTMC) and other secondary/tertiary healthcare systems; doctoral practices, Chambers of Commerce, free standing IL/AL, social/business groups, senior centers and churches in the identified geography.…
