Location: Eidi Family Rehabilitation Center Department: Rehabilitation Weekly Hours: 40 Status: Full time Shift: Days (United States of America) Job Summary: As the RN, Rehab Transition Care Manager, you manage all aspects of patient transitions to and from Flower Rehab Center. This role will work closely with physicians, nurses, therapists, leadership, patients, caregivers and the community. This role involves extensive medical record review and you will actively participate in team meetings and education regarding admission requirements and processes.
