The Social Services Coordinator is responsible and accountable for assessing the psychological social emotional cultural or financial needs that impact a resident's health and recovery. To provide professional social work services that utilize supportive counseling and clinical knowledge in assessing the needs of patients and families during illness crisis intervention and post discharge planning.
