Organization: SRMC-Sutter Roseville Med Center Position Overview: Conducts clinical evaluations, assesses patient referrals, and facilitates admissions through collaboration with other rehabilitation disciplines and physicians. Serves as the primary contact for referrals from physicians, external agencies, hospitals, case management (internal and external), and caregivers, including family members. Fosters and builds referral network by educating resources on the advantages and benefits of Sutter's Acute Rehabilitation Program, executing ongoing marketing activities, and developing working relationships with hospitals, businesses, allied health professions and community-related groups.…
