About this role
We are seeking a Physician Assistant or Nurse Practitioner to join the M Physician’s Memory Care team. This is an outpatient role and will be practiced at the M Physician’s St. Louis Park location. Practice Details: Conduct comprehensive cognitive assessments, including history-taking, neurological exams, and validated screening tools. Develop and adjust individualized care plans in collaboration with your Memory Care neurologists and others on the treatment team. Provide education and counseling to patients and caregivers on disease progression, treatment options, and community resources. Respond to urgent or emergent neurological issues related to prescribed treatments, such as anti-amyloid therapies, to evaluate and triage next steps.…
