The role empowers individuals through recovery-focused care, develops personalised wellness plans, connects clients with services and community resources, and collaborates with families, health professionals, and stakeholders to promote wellbeing, independence, and positive recovery outcomes Key Responsibilities: Complete clinical assessments and provide mental health support for Pacific people and their families.
