Position Summary To engage patients and families in identifying psycho-social needs in order to develop appropriate interventions and coordinate optimal, and safe plans of care. To educate, counsel, and refer as indicated. To plan and communicate effectively with multi-disciplinary team and, community resources and support the Information Sharing Core Element of PFCC.
