Social Worker, Social Services, Full Time, Day

Position Summary To screen, assess, and evaluate patients and their families to identify the social and emotional impact of patients living with a terminal illness. To develop interventions to increase coping with an impending loss and continuously assess for comfort and safety as the patients and caregivers needs change. Education Master's degree in social work from an accredited program required, New Jersey Social Work License required. Experience Previous home hospice social work experience preferred.
