This position works as part of a multidisciplinary team with social workers, nurses, healthcare providers, and community partners to improve maternal, infant, and family health outcomes. This is a full time (40 hours per week) , benefit eligible position typically working Monday - Friday with our Strong Beginnings program based out of our Westside Health Center. This is a home-visiting program. Some weekend or evening hours may be required depending on the needs of the program. Certified CHW is preferred.…
