Job Description A bit about this role: We want to help patients navigate the healthcare system in a better and safer way, feel their best, and better manage their chronic diseases. Our transitions of care program is critical to achieving these goals, and TOC Coordinators will play a key role in supporting these programs. As the supervisor for the TOC Coordinators, you'll lead a team of TOC Coordinators in serving as advocates for these patients, supporting the TOC clinical team. The goal is to ensure the patients have the necessary resources and support to transition to home from the hospital and/or post acute setting and prevent re-admissions.…
