Day (United States of America) Intake Care Specialist - Care at Home The Intake Care Specialist performs a variety of duties. This position requires skill in planning, organizing and coordinating all incoming referrals from outside sources. This position is also responsible for scheduling of Nurses, Therapists, and Home Health Aides for a specific patient population. This position reports directly to the office manager and assists patient care coordinators and office staff as needed. - High school diploma or general education degree (GED) - One to three years related experience and/or training; or equivalent combination of education and experience. - Ability to communicate with a wide variety of individuals and families of varying life styles, ages, cultural and ethnic backgrounds.…
