That is the work here. As a Social Worker with Veteran Pathways (part of the Proactive Health Group), you are the person who holds a client's care together from day one. You will run biopsychosocial assessments, take your seat in GP-led case conferences, shape the care plan, then walk alongside your client through a structured 90-day journey.
